In order to get the most out of our time together, Please fill out all forms and send them in advance.

New clients must download the forms here, fill them out and email to robin@drrobinshaw.com 24 hours prior to your first appointment date unless otherwise notified. Alternatively you may print and bring completed forms to your appointment.  

These forms require the free Adobe Acrobat Reader software to fill out and complete electronically.

Payment Info

Payment is due at the beginning of each visit.
You may pay for your visit with cash, check or credit card with a processing fee.

FEES:

Fees vary with the different treatment options offered. Please contact our office for more details.

Sliding Scale for self pays

INsurance:

At this time, we only file with Seton Health Plan. It is possible that your insurance may cover part or all of your sessions, however, you must file directly with your provider.
 

Cancellations: 

We require 24 hours notice prior to the scheduled appointment time for any cancellation or rescheduling of appointments. If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session. Changes without 24 hrs notice will be billed at our full rate.
